Direct billing Guideline

 

Please bring your insurance card on your first visit. Some benefit plans require you to submit a prescription for massage therapy from your family physician. You must submit the prescription to your insurance company (not to me) before we set up direct billing for you if this is a case. If you are not sure about requiring a prescription, you must call your insurance company PRIOR to your first visit. We do not have access to your personal information about this from your insurance company directly. If the insurance company requires prescription from your doctor and you don’t submit this, your claim will be rejected through direct billing.

 

For your first claim, we will submit all the documents for the claim through the direct billing portal. We will request the company to pay you this time. You will have to pay me upfront for your first visit. I accept payment by cash, cheque, debit, VISA and MasterCard.

 

For later claims, we will submit the claim and you will not have to pay from your pocket if you are covered 100% or you will pay only the portion that your insurance does not cover. For example, some benefit plans only cover 80% for your treatment fee. In this case your pay us upfront 20% of the fee.
